About Chronic Disease Prevention and Control

Each year chronic diseases are responsible for 7 out of 10 deaths, and treating people with chronic disease accounts for 87% of our nation’s healthcare costs. Public Health – Idaho North Central District is committed to reducing the burden of two major chronic diseases: Diabetes and Hypertension – by encouraging screening, building partnerships, promoting media campaigns, providing healthcare provider education, and attending community-based programs and events.

Healthcare Provider Education

Public Health- Idaho North Central District partners with healthcare providers to help them improve the prevention and control of hypertension and diabetes in their practice. Because complex chronic conditions require both high quality clinical care and effective self-management Public Health addresses improvement in two domains:

  • Domain 1: Healthcare Systems Change – Promoting resources and interventions with healthcare providers to improve prevention, early detection, and management of hypertension, diabetes, pre-diabetes.
  • Domain 2: Community/Clinical Linkages: Increase healthcare provider referrals to community resources that support chronic disease self-management.
